Requirements:

• Letter stating goals; reasons for request
• Letter of acceptance to medical school for the coming academic year
• Scholastic performance
• 2 letters of recommendation from professors
• Involvement in extra curricular activities and athletics
• E-mail address

The amount of the award shall be flexible and dependent upon available funds.
